Unfortunately yesterday nobody had any tasks for me and I was left with 3-4 free hours. Today I will have even more free time so the offer still stands:

🔔 Tragédie v Rusku. Vlak narazil do zříceného mostu: mrtví a ranění. Podezření na „zásah zvenčí“:
V ruské Brjanské oblasti u hranic s Ukrajinou se v noci na dnešek zřítil most, do něhož následně narazil osobní vlak a vykolejil. Nejméně sedm lidí zahynulo, uvedl gubernátor oblasti Alexander Bogomaz. V nemocnicích skončilo 44 z 69 zraněných lidí. Zraněny byly i tři děti, jedno těžce. Podle agentury RIA Novosti je mezi mrtvými i strojvůdce. Podle úřadů se most zřítil v důsledku zásahu zvenčí. Bogomaz dodal, že most se zřítil ve Vygoničském okrese v místě, kudy vede federální dálnice.

After attending a conference of major pharma yesterday with their sponsored physicians and dinner, there are things I no longer fully understand anymore.
When I was a resident years ago, the LDL cholesterol targets were <100-120, The blood pressure goal was <140/80 in dyslipidemias and hypertension.
Today the targets are less than <70 and <125/85.
I wonder if in a few years the goals will be to have zero LDL cholesterol and no blood pressure.
The patient will be dead but will be in good health for sure!
Maybe I'm getting old, but we need to read the scientific evidence between the lines and have different standards of objectives as the medical community. Take care of the patient's life, encourage them to exercise, to a healthy diet. Take care their pockets and wallets from big pharma. All in an unfavorable environment for us.
Medications will be used when necessary because I am far from being a naturist.
But for me someone who comes after 3 months of effort, exercise, diet and a minimum of medication, managing to control blood pressure that was not controlled before and now at 130/80 and LDL cholesterol through the roof before and now at 110 deserves a party at the clinic and then we will see more inflexible objectives later.
We can't be so square as to not understand that.

I actually don’t hate Elon at all. His public presence is so unbelievably suffocating if you are paying attention to him as a public figure that I just try to not pay attention to him or what he is doing in public as much as possible. This has been a good balance for me because I don’t dislike him. I’m neutral. I can actually appreciate aspects of his work and companies when I see it if it’s good. If it’s not good, I shrug and move on with my day.
As far as twitter goes, the censorship was so bad that I came desperately looking for nostr. X isn’t censorship resistant either. That specific issue couldn’t be resolved even under new leadership. I’m grateful that people understand that the censorship on twitter and other corporate tech providers was wrong. It was wrong and it’s still wrong.
What has made X so unbelievably insufferable was their monetization program. It incentivized horrible content. The vibe fell off. So you can technically mostly say whatever on X, if the platform is running smoothly, if the government hasn’t turned off access or done a takedown request BUT the vibe is off because rage-bait, boomer content is incentivized. It’s freeish speech to a bunch of bots and even then you have to pray that you are algorithmically blessed to get an opportunity to speak to bots. The juice isn’t worth the squeeze.

🗣️ According to Perplexity, here’s what’s happening and why it matters:
Credit default swaps are essentially insurance policies against government default.
When you see CDS spreads rising, it means investors are willing to pay more to protect themselves against the possibility that the U.S. might not be able to pay its debts.
