I'm working on the doctor issue. But, just to add on the brokenness issue. I lived in Serbia for a couple years. That is a poor country in Eastern Europe. They provide better public health care than Canada, particularly for kids.
A country that is small and far less wealthier than Canada provides better care.
I'll contrast this with Australia, where I lived for seven years. And I'll say, they have much the same problem as Canada.

Trump names the judge and law clerk from the NY civil frsud trial, also, showing their faces, on his web site Truth Social. Whatever his motives, it's clear this is a direct threat of instigating violence against the judge and clerk. The 1st Amendment is not a suicide pact, nor a rationale for threatening officers of the court.
This strikes me as very dangerous.

Lemmy, as a member of the FediVerse community, is deeply broken. It promotes toxicity copied straight from Reddit, and further violates the Charter which embodies the purpose of the FediVerse to begin with.
As an example, I have been banned at Lemmy.ml for challenging the use of the slur "shitlibs" in debate. Saying that it's a slur, no different from any other slur, and paints whole peoples in a negative light for their beliefs.

Doesn't make them any less of an abusive behemoth to users. They are. But unnecessary complexity is crucial to understanding why FOSS solutions fail with users.
Compare Blender before the 2.8 interface redesign to after, and see how that impacted adoption. It wasn't features, it was usability.

Number 2 is particularly interesting because you'd expect Elon would just want to boot his political opponents. But in fact, it helps with engagement. Because it creates conflict and anger and this is known to keep users active on the site.
It also presents the opportunity to turn his opponents into supporters over time with conditioning.
You have to imagine this kind of process running for months or years on someone. It's like automating the shifting of the the Overton Window among users.

I think the place to start here is the advance of Large Language Model AIs. This has allowed Elon to fire a lot of moderator staff. He foolishly fired engineers too, but that's an aside.
The AI software can read a tweet and understand some context of its content and subtext. It can then use that to tag and rank the tweet based on its content and not just who wrote it.
Twitter already knows users' ideology based on likes and retweets. But with an AI, it can granularize reach by content itself.
