If your team / business has had a critical or breaking incident and diagnoses a "root cause" *while* they're still working through restoring services, not only will you continue to have the same type of major incidents, but your team will not be learning what they can do to improve and how to be more resilient to failure in future.

I have pretty mixed views about NPS. It's a crude metric but I can appreciate some stakeholders opting to defer to a blunt tool for expediency's sake. But as tools go, there isn't really a strong basis for a "cause-and-effect" relationship.
How you gather your data, who you include in your pool, what your response rate is... these can all be pretty arbitrary. Survivorship bias, anyone?

This is true, and there's a lot to feel encouraged by in what we're seeing, but also that's a concern in itself.
The attacks on Māori, te reo, Te Tiriti are coming from all quarters. The government is engaging in an orchestrated, concerted and multi-front offensive on all things te ao Māori to the detriment of us all as a country. And it's exhausting. There are so many battlefronts right now that the goal is clearly to overwhelm by exhausting / choking resources.

Documentation is actually quite hard. It's a lot like gardening in some respects: it's complete, but never "done".
Putting things in a logical structure, with a logical hierarchy, in a way that's cross-referenced and findable is actually quite an impactful exercise.
