When I come across a nasty note in a thread I mute that user, though I've only done that a few times.
There's a quote that I like about free speech that I'll paraphrase - Free speech is horrible, but it's better than the alternative.
No doubt there is terrible stuff on here, and as Nostr gets more popular there will be even more (whether genuinely horrific people or just trolls trying to give Nostr a bad name). The Beave (npub1q6y…t3sh) 's suggestions about relays and words/hashtags are great, and there will surely be more options to curate your own algorithm over time.
