Measles isn't just a skin rash. Among other complications, it causes Subacute Sclerosing PanEncephalitis, which is fatal. SSPE has a 4–10 year latency to manifest following infection.
You will see:
- Progressive neuropsychiatric issues, seizures, visual impairment and movement disorders, with myoclonus being the most common associated movement disorder.
- Chorea, dystonia, tremor and tics may occur. Parkinsonism and ataxia may also manifest.
- EEG may have classic periodic generalized discharges.
- Look for anti-measles antibodies in the spinal fluid.
Until recently, practitioners in the US rarely encountered measles related symptoms in the neurology clinics and on the hospital wards. Now that the number of folks receiving vaccination for prevention of this disease has decreased, we are observing a resurgence of measles; and of course in some cases the delayed manifestations of SSPE.
