Its how the client is fetching followers from relays, I think followers are not in one's account, the ppl you follow is. So going to your relays fetching the profile the ppl you follow is there, but the ones that follow you doesn't, the client had to figure it out in which relays your follwers might be, because the data saying that a person follows you is in that person profile, and he can be in completely different relays from yours. This is why the ppl you follow is always right, but the ppl following you can be off in any client. This is how the protocol is, some clients can handle this better than others. One client where you can get a good overview is nostr.band its a site you can even go to your profile there and see the actual json of your profile.
