My shitty but subsidized ACA health insurer just sent me a 16 page letter through the mail to tell me that a 15 dollar charge had not been paid to my provider because it was billed on the incorrect form.
Nowhere in those 16 pages did it say what the charge was for, although it did reference a "line number" in a document not otherwise identified.
