The midwife should be there as an advocate (if you already paid for her), don't expect a refund just because you need a hospital. At a minimum have a doula with you. Do not use the "hospital supplied" midwives or doulas, the incentives do not permit them to do their jobs. Make sure you are spirituality compatible with your midwife or doula, the last thing you need is to get into an argument with them. During labor, a born-again women can instinctually sense spiritual darkness in people who are nearby.
