I knew someone from Liverpool who pronounced "health" as "elf" (we tend to drop our Hs, and "th" said like "f" isn't uncommon).
Her husband, who wasn't from Liverpool, asked her one day, "Who's this elf you keep going on about? And loads of other people, too? Why do you all say you've got an elf?"
He was confused by the phrase "I'm not doing it for the good of my health!"
