I've seen it happen, but I think nothing in that logic has changed in latest releases.
Looking at the code now I don't see any obvious bugs.
My first guess would be that you were waiting for the signEvent() promise to complete but it got canceled for some other reason first? I don't even know how that is supposed to work on JavaScript.
