I'd guess that larger organizations wouldn't fall into this specific trap in the first place if for no other reason than they'd not update and reboot everything at once. For the scenario mhoye talks about, I'd think the most vulnerable org would be a smaller shop with just 1 to 3 servers. A real possibility someone might decide to update everything at once. I'd guess a larger percentage of smaller orgs also didn't plan for offline encryption key backup.
