It’s a human weakness that we can never model a complex system in one go. It has to be iterative and we have to acknowledge that whatever system we produce will have issues and they need to be fixed.
It seems very weird to me that this was not the default state with Fujitsu systems. This is the default mindset in all projects I have worked in the it sector.
This also applies to nhs and any other organisation that thinks technology will solve all their problems.
