I think it's even worse than that. Saline is safe, but something going straight into the bloodstream is a pretty big deal for the body. We have many layers of skin to prevent that from happening, and the only other times something gets in that deeply is a crisis: a bite from a snake (or other creature) or a bad wound.
I'm not sure it's actually possible to have a true placebo for an injection.

But -yes - saline would be a good start!
