I looked up the form and apparently it’s common in all states to ask about menstrual history because irregular menstruation can be a sign of ill health, especially for athletes, but the info on the form is still covered by HIPPA and school officials don’t meet that criterion. Well-intentioned or not this information shouldn’t be collected on a school form.
