I feel some context needs to be provided to my previous toot.

Typically, in Doom, item collection % only refers to "artifact" items. These are items you can pick up regardless of your health and ammo status. So health, armour bonuses, backpacks, and certain power ups. Might be slightly wrong there, YouTuber Decino has a more detailed breakdown on this stuff (And other Doom mechanics).

Having 100% item collection involving ALL collectables would mean you'd have to strategically take damage to pick up med kits and stim packs and waste ammo to collect everything. Thankfully, 100% item collection achievements need to be done once per chapter (every 8 or so levels)
