It's actively managed and the admin is selective (like on a WoT relay or topical relay, or someone's personal relay), it's paid (like nostr.land), and/or has relatively strict policies in place to limit misbehavior (like damus).
I don't directly post to nos.lol, for instance. I've had terrible experiences with that.
