According to Wikipedia the earliest known proto package manager is SMIT on IBM AIX..
But the earliest true package managers are either dpkg/apt of Debian or the Port system of Freebsd and other BSDs are the oldest package managers to exist... Both roughly came at the same time in 1994.. dpkg/apt was actually a big innovation developed by Ian Murdok the founder of Debian Project which streamlined and automated the entire process of dependency hell. RPM came one year later in 1995.
