I think the Core Technology Fee feels like such a sword of Damocles, because Apple controls all the factors that could cause you to hit that threshold.
If they wanted to, they could promote your app until it crosses that threshold, and they control automatic app updates, and whether apps are offloaded from a user’s device.
I don’t really think they’d ever weaponise this, but it shows how perverse the incentives are.
Growth becomes dangerous, and that’s grim.
