Generally, I see it used when someone is so concerned with minutiae and technicality that they ignore broader, more pertinent context and, typically, when they do so deliberately. In cases like this, it absolutely carries a deservedly bad connotation.
I don't think that's the *only possible* usage of the word, but it's virtually always the usage I come across.
