Dang!!! I ended up winging this batch but I generally followed this video (I made it exactly last year and it was delicious).

1. I slice the tomatoes (green and red) and bake them for about 25 minutes with olive oil, garlic, jalapeños, poblano, habanero, bell, white onion, salt and pepper.

2. Mash it up a bit and add to a pot over heat. Add a touch of apple cider vinegar, cilantro, salt/pepper and simmer for a while until soft.

3. Blend in food processor (or just mash) until it is your desired consistency. Then can it and boil until sealed (if you want to store for longer periods).

Will stay good unopened for at least a year. I still have some from last year and it is still great.
