One theory I read somewhere is that he contracted some STD while going to a prostitute in his youth, and that it was contributing to his madness. I can also see how that would have affected his views on hedonism as well.
I also think, that Nietzsche at that time was part of the "nerds" being so intelligent, which would make it more difficult to meet women.
