Mastodon DMs are not end-to-end encrypted. In theory a server owner could manually see DMs in their server's raw database.
If you want encrypted private communications, I'd recommend XMPP (aka Jabber). It is federated into servers like Mastodon, but is designed for encrypted messaging & calling.
