NostrGram uses a caching server. It reads from that server, not specific relays. The server gathers notes in real time from the top 100 relays and it writes to the top 25 relays. You can configure custom relays to write to in the settings (e.g. the paid relays) and you can specify which relays to read the Global feed from (or enable reading only from paid relays on Global to remove all the spam).
