So does also run a relay but the client (the app) and relay function completely independently.

The Nostr protocol doesn't have a way to natively store or send binary data, like photos, videos, etc. so there are other services like nostr.build or void.cat that have specialized to handle that aspect for apps. So Damus, Snort, etc. all use on of these services when you upload a photo.
