We know why (Dudes and Dudettes), because a cyber security exercise becomes cyber warfare in the instant someone decides to leave their scope (which could be a single command).
Most of the time it's "accidental" and it gets reviewed with the client.
Conducting exercises in "production" can be justified.
But for such important events there should only be air gapped monitoring of the inputs and outputs to make sure they're correct.
