Functionally I think the holes are there so the baby can't get good suction - they suggest you cut the tip off pacifiers to prevent successful sucking when you are weaning your kid off of them. So it is kind of pointless to have a design without the holes unless you are just giving your baby an even cooler thumb to suck?
