some solutions more recent projects seem to have for this are:

* markdown + pandoc, for example: https://github.com/eza-community/eza/blob/edcf392c78abd8abea76d4f59fb639ad134a0d8b/man/eza.1.md
* custom code to generate the man page (curl & ripgrep do this, curl defines its options like this https://github.com/curl/curl/blob/2dbe75bd7f3c36837aa06fd87a442bdf3fb7faef/docs/cmdline-opts/curves.d)
