It usually doesn't (the "major" version getting big, that is).
The thing that *does* matter is the frequency of breaking changes - and if a project releases breaking changes every 2 months (be it pre-1.0 or post-1.0), that's not a good sign in my experience ...
So when I see a library that's only 2 years old and already at version 12.0.0, I'm just a little suspicious.
