Fake engagement spam is probably Nostr’s biggest downfall, it is ridiculously easy to create many fake likes and follows, and messages. I experimented with this a while on a NPUB created for this purpose, I was able to create thousands of fake likes and messages, I did it very briefly as not to upset anything, but this could get very bad, some relays try to limit repeated stuff from the same NPUB but it’s easy to generate new ones too. I don’t know of a way to overcome that.
Same goes for Ai bot spam.
