The 'sins' have come from those that care about their own motivations, paying for things using money from those that they don't actually care about - an overly broad generalization, but we're just some amorphous blob of 'people' providing funds. Their obligation however is to also use that money to provide assistance in return to that amorphous blob of individuals with conflicting interests, so there is less incentive to keep the quality high at a high resolution when they don't actually see the broad spectrum of individuals and their lived experience.

Its okay to have centralized leadership, but the system breaks down when its communication channels fail (or when parts of the system choose to not adhere to surrounding system signals). The brain in charge of the nervous system functions very well to keep the body alive, but it must also trust that the body can do what it expects through the signals it sends. We built governments to act like centralized control imposing will and dictating actions, but natural systems operate differently. Signals are sent down a chain with trust that they will effectively know what to do, but that presupposes good communication channels and good trust in the system's components to act.
