Better to offer options (eg mark as NSFW, keep private, etc) than trying the moderation whack-a-mole you're gonna lose in the end anyway. Another option is crowd moderation. Trickier and délicate to set up, but arguably best efficiency and scalability. Anyway, don't feel bad, it's the early days, everyone has the right to make mistakes and experiment.
