They skate around that by having doctors on staff and they determine which procedures get approved without arguing and extra work by the medical office.
It's ridiculous and doctors have to have large staffs to handle this. And the insurance companies are staffed by people who can legally block a procedure without further input from the doctor's office.
We have a terribly f'd up healthcare system in this country (USA).
