Dear ,

We are writing to inform you of a ransomware attack against [REDACTED] that may affect the privacy of some of your personal information.

What happened?

On [REDACTED], we became aware of deployment of ransomware in our legacy computer system. That same day, we activated our incident response plan and took steps to contain the incident. We also promptly began an investigation with the assistance of third-party cybersecurity experts to determine the nature and scope of the activity, assist with containment, and ensure no ongoing unauthorized activity.

Our investigation determined that one user account, one computer, and one legacy Network Attached Storage device (NAS) had been compromised by the attack. We were not able to identify the date that the ransomware was installed. To remediate this vulnerability, the public link was immediately severed, and the computer was taken offline and decommissioned.

What information was involved?

The information that may have been accessed included your name, mailing address, phone number, email address, date of birth, social security number, and/or government issued identification number.

What are we doing?

[REDACTED] takes this event very seriously.

To help relieve concerns and restore confidence following this incident, we have secured the services of [REDACTED], a global leader in risk mitigation and respons, to provide identity monitoring for 12 months at no cost to you.
