(b) That's a list of things they say they "may" collect. They can't get any of it without your permission through iOS. They don't even prompt for your Health data, but even if they someday do, you can just decline. They don't ask for your location data. If they do in the future, you can decline. Etc.
The rest is just what they collect through your using the app.
