What client do you use? Im an amethyst user and here we have the note-comment bug that occasionally posts new notes as comments on the last note that you have commented on. The only way to avoid it is to clear the app's cache before every new note. It has been reportedly fixed once but continued to happen for me, so I guess I'll never trust the updates and always clear the cache before every new note, forever 😅
