This isn't an area I've studied, but from a brief search on papers, it seems like mortality rate only really got under control in "big" cities around 1800 or so, so the allure of cities was enough to draw people in despite the mortality risk.

I don't know if a city the density of Tokyo could really exist without medicine, but I think there'd still be people generating a lot of wealth in cities with higher density than we had in 1800 and higher rates of mortality from disease.
