You nailed it. I think it's a law of internet physics that something like outbox will only happen if it has to happen, if there is no lazier alternative.
Realistically it's just too easy to crawl and index all of nostr, or to make use of someone else's index, or to just lean on the big relays. Since there's no way (other than scolding) to prevent any of those, I think this internet physics means outbox is pretty much impossible. Too many players will see it as unnecessary extra work (and they'd be sort of right).
In a nostr-wide sense anyway. It can definitely work in an closed-off and controlled environments.
