Brown recluses are an interesting thing. They can't actually bite you on their own (most spiders are like that actually), and instances when they do bite are because of the exact situation you described. Spider in bed sheets (or in clothes), it gets pinned against your skin and the pressure of you or your clothes against the spider is enough to initiate a bite. Otherwise they'd just run away.
The other thing is that their bites probably are not as venomous as we once thought. What's probably happening is the necrosis people report is actually a secondary staph infection after the bite, which people attribute to the venom itself. So long as the bite it kept clean, the effects are pretty minor. This is usually the extent of their bite after a week.
