I think there's actually a lot of potential here.
Maybe it takes place in a time/place where most people are genetically modified clones. They are all born "neuter"—without any gender—as society treats them as disposable workers and has no need for them to breed nor express a gender.
However, the human desire for gender expression cannot be suppressed, so an underground genderpunk movement exists to provide hormones and surgery.
