I'd probably not bother with 1.2V on there because at that low a voltage you really want to regulate it onboard to avoid droop from ohmic losses. it's easy to regulate onboard from 3.3V too.
I'd go with something like TX, RX, GND, 3.3V, 3.3V, 5.0V, 5.0V, 12V, PRSNT#, PG
the device pulls PRSNT# to GND to indicate that something is plugged in. that's useful if you end up putting load switches on the supply to help manage inrush current.
PG is useful too for DC-DCs that have a PG out.
