I think we need to think about things on a case by case basis, probably. Sometimes stepping in to help actually puts pressure on the institutions themselves - for example, the Black Panther Party's Free Breakfast for Children program pressured the government to introduce aid programs of their own as counter-propaganda.
On the other hand, you are right that when our communities, or our conception of our communities, encompasses people already in a position to support themselves, it can result in greater inequality.
I suppose this is a big reason why I wish there was less class and race segregation, but given that there is not, you are spot on that this could be a real problem with certain kinds of community-driven programs.
