The alt text is in the alt attribute, which I can't see at all, using a graphical browser with images enabled. I had to write a custom userscript for oglaf.com to make the alt text more accessible to me because every comic there uses both of these (semi-obscure) HTML attributes.
So ... most people have probably no idea that there's hidden jokes and commentary everywhere. 🙂
