the difference is that fediverse's protocol is designed as server-to-server protocol because servers are supposed to take care of everything for clients
nothing is stopping you from making nostr like that, sure, but it just reminds me that there is always a contingent of people that want to make a system something that it is not, eg the big blockers that want to make bitcoin like large-scale payment system that runs on a few large servers in datacenters, instead of trying to keep a lid on required specs
the problem of this is that you're spending a lot of work making a (possibly worse) implementation of something that already exists
