I was heavily into Risk in various forms in the final part of my NHS career - I was responsible for writing many risk assessments and investigation of incidents where 'bad things' happened.
I worked closely with a respected colleague who was H&S trained.

It was our considered opinion that kneejerk reactions (often in the name of "Health & Safety") often had the opposite of the desired effect and only by slow, careful consideration of the real facts could mitigation be put in place.
This mitigation was often - in my world - a review of training needs, or making sure people had the training that was already supposed to be in place.

It's a crying shame that "Health and Safely" is still used as an excuse to not do something rather than to find out how to do something safely.
