Your notes are signed by your private key, and the signed event id is a hash of the content of the event, any change to the event content would be automatically detectable the signature will simply not match the content.
Also there's nothing stopping relays from talking to each other, in this case the relay simply acts like a client.
There is no consensus needed since each event is independently verifiable.
