talk directly to another client can be a security issue too. Because you have to "open" door to it. That's why many messenger use an external relay to not "push" things but get them from them.
It is the same when you host a bitcoin / lightning node or #nostr relay. You have to allow some incoming stream.
There is no perfect solution here, only a best choice between privacy and security issues.
