They're called "Small Diameter Bombs", SDB's for short. Israel primarily uses the GBU39. Large buildings are made of lighter material, otherwise they would collapse under their own weight. Toppling them is pretty easy. Israel does it intentionally, because if you hit it hard enough to implode the way you want it to on the first hit, there's a chance it could fall into the street or domino adjacent buildings.
Yeah, I guess it's feasible the clinic was packed, but the pictures of the aftermath show no structural damage to the place itself. I don't see how 500 people died in a parking lot and Hamas was there to count all of them and call it within 15 minutes.
The whole thing appears to have just been a scam.
