I paid $3.89/month for a period of 3 years to Fastmail to get all of the features you mentioned, and more. I doubt there's anything cheaper out there with all of these features and without annoying limitations. For example, Migadu limits outbound emails to 20 per day on their cheapest $19/year plan. Posteo is cheap (€1/month) but they don't support custom domains and drop emails they think are spam. Mailbox.org seems to be an option as well at €3/month although I have no idea about their service quality. mxroute.com offers email at $49/year although it doesn't seem to offer a trial.
You can try using Fastmail.com and Mailbox.org for 30 days without paying anything and judge them both. mxroute also seems to be an option. If you decide to go with Fastmail and want a 10% discount for the first year, let me know.
