I had a quick look at the code in that branch and `instances` there appears to be a *view* (not a table!) and indeed does not have a column called `id`, just the domains and numbers of accounts on them.
Moreover, there doesn't seem to be any code there managing the index of instances, judging by the absence of anything with InstanceIndex (unless I'm missing something).
Is the code where this happens entirely from that branch or mixed with something else?
